Low back pain is a common reason for consultation in general practices. Despite the evidence and consensus-based recommendations of the NVL for unspecific low back pain, the adherence to the guidelines is low and partly justified by patient expectations. The aim was to investigated the extent to which patient expectations correspond to recommendations of the German national guideline for management of low back pain and whether patient characteristics, history of LBP and previous treatment experience are associated with expectations. A cross-sectional study including 1018 patients from 13 general practices was conducted. Data on demographic, medical history of low back pain and the expectations and beliefs regarding the management of low back pain were collected using a questionnaire. Inverse probability weights were used to assess non-response bias. Descriptive analyses as well as univariate and multivariate logistic regression models were performed. A total of 977 patients with a median age of 57 years and 60% women were included in the analyses. 66% of the participants were willing to forgo further examinations, 44% expected imaging despite the lack of an indication,70% expected a massage, 44% expected injections and 67% agreed to continue their everyday activities.
