Korean Abstract: 본 연구는 고령화연구패널 자료를 활용하여 우리나라 중고령 사망자의 생애 마지막 3년간 본인부담 의료비(OOP) 규모를 분석하고, 본인부담 의료비가 어떤 항목에서 많이 발생하는지와 본인부담 의료비 규모에 영향을 미치는 요인을 함께 살펴보았다. 고령화연구패널의 사망자 설문(exit interview)에 포함된 대상 가운데 조건에 맞는 1,042명을 최종 분석에 포함하였으며, 사망일을 기준으로 거슬러 3년간 해당 대상자를 위해 지출된 본인부담 의료비를 계산하였다. 분석결과 사망 전 3년간 발생하는 본인부담 의료비는 평균 855만원이었으며, 중앙값은 이보다 낮은 448만원이었다. 연구대상자의 26.4%는 1,000만원이 넘는 본인부담 의료비를 지불하였으며, 전체 연구대상의 8% 가량이 사망 전 3년간 당초 보유하고 있던 가계순자산보다 더 많은 의료비를 지불한 것으로 나타났다. 회귀분석 결과 사망 전 본인부담 의료비에 상대적으로 가장 큰 영향을 미치는 요인은 투병기간이었으며, 사망 전 보유한 만성질환의 유형 또한 중요한 영향요인이었는데, 특히 암질환 여부가 본인부담 의료비에 결정적 영향을 미치는 것을 볼 수 있었다. 본 연구결과는 향후 개인가계의 노후설계 및 관련 교육, 정책 수립의 근거자료로 활용될 수 있을 것이다
English Abstract: This study examined total out-of-pocket medical expenditures(OOP) in the last three years of life in middle aged and elderly households. Using the KLoSA dataset, which is a nationally representative panel survey focused on middle aged and elderly households, we conducted retrospective analyses of the total OOP over the last 3 years of life. We identified KLoSA decedents between 2009 and 2016, and defined a 3 year study period using each subject's date of death(n=1,042). The average OOP in the 3 years prior to death was 8.55 million KRW(median: 4.48 million KRW) for each decedent. Overall, 8% of subjects' expenditures exceeded the baseline total of household assets. Regarding cause of death, decedents that died from cancer spent the highest OOP, with average spending of 12.95 million KRW. This study demonstrates the cumulative financial risks arising from OOP faced by older adults, particularly near the end of life
