  • Description: <jats:p>There is a well‐recognized echocardiographic pattern of the right ventricle (RV) wall motion reported by McConnell which is characterized by normokinesia of the apical segment and akinesia of the mid–free wall with persistence of abnormal wall motion at the base. We describe a case of an 85‐year‐old man with acute massive bilateral pulmonary embolism with opposite regional contraction pattern of the RV free wall than the one referred by McConnell consisting of hyperkinesia of the basal RV free wall and akinesia of the mid‐RV free wall and RV apex, a finding that could be called “inverted McConnell sign.”</jats:p>
