In medical terminology the term „transition“ denotes the purposeful and planned movement of adolescents with chronical diseases from a child-centred to an adult-centred health-care-system. Many clinical models have been developed yet to improve the medical care in different pediatric subdisciplines during transition. At present children and youths with chronical neurological diseases or motoric and mental disabilities are treated at SPZs (Sozialpädiatrische Zentren) by the age of 18.
