    This study explored the benefit of a smartphone-related pulsewave analysis for detection and prediction of a coronary artery disease (CAD). The autonomic nervous system has a strong influence of the regulation of the cardiovascular system. One marker of this influence is the heart rate variability (HRV), its analysis is a known instrument for diagnosis, risk stratification and prediction of different cardiac diseases like CAD. Classic HRV-measures depend on short- or long term-ECG analysis. In this study the peripheral measured pulsewave variability (PRV) was observed, which has a high correlation with the HRV. 167 subjects with suspicion of a CAD and indication for elective coronary angiography were examined by a smartphone-based record of the pulsewave over six minutes. Out of this data the PRV was generated. The frames of the coronary angiography were retrospectively observed and eventual stenosis in eight coronary segments quantified. CAD was defined as ≥50% luminal diameter stenosis in at least one of those eight segments. It shows that PRV-analysis can help in diagnosis of CAD in patients with sinus rhythm. This study showed that PRV-analysis can use as a method of elimination in patients with CAD-suspicion. To use it as a screening tool it seems not ideal regarding sensitivity and specificity.
