Here, we provided a case of a 19-year-old man presenting with 2 intra-abdominal masses, who had a history of intracranial germ cell tumors and a ventriculoperitoneal shunt implanted in order to alleviate obstructive hydrocephalus. The intra-abdominal lesions showed intense FDG uptake and were confirmed to be germ cell tumors by postoperative histopathology. 18F-FDG PET scan could be useful in monitoring metastases of intracranial germ cell tumors via ventriculoperitoneal shunt.
