<jats:title>Abstract</jats:title><jats:p><jats:italic>Objective</jats:italic>. To examine the relationship between α<jats:sub>1</jats:sub>‐antitrypsin (α<jats:sub>1</jats:sub>AT) specific activity and tumor necrosis factor α (TNFα) concentration in synovial fluid from 48 patients with rheumatoid arthritis.</jats:p><jats:p><jats:italic>Methods</jats:italic>. The specific activity of α<jats:sub>1</jats:sub>AT was calculated from the measurement of α<jats:sub>1</jats:sub>AT concentration (by rocket immunoelectrophoresis) and elastase inhibitory capacity. TNFα was detected by enzyme‐linked immuno‐sorbent assay.</jats:p><jats:p><jats:italic>Results</jats:italic>. TNFα concentrations correlated with the extent of α<jats:sub>1</jats:sub>AT inactivation.</jats:p><jats:p><jats:italic>Conclusion</jats:italic>. Our findings are consistent with a role of elastase in TNFα release within the inflamed joint.</jats:p>
