<jats:title>Abstract</jats:title><jats:p>Over the past few decades, <jats:italic>Streptococcus dysgalactiae subspecies equisimilis</jats:italic> (<jats:styled-content style="fixed-case">SDSE</jats:styled-content>) have been considered as weak pathogenicity compared with <jats:italic>S. pyogenes</jats:italic> (<jats:styled-content style="fixed-case">GAS</jats:styled-content>). Some recent reports argue that <jats:styled-content style="fixed-case">SDSE</jats:styled-content> may bring severe soft tissue infection as same as <jats:styled-content style="fixed-case">GAS</jats:styled-content>. No reports have been tried to reveal the clinical characteristics and autopsy images of fulminant <jats:styled-content style="fixed-case">SDSE</jats:styled-content> infection. In this case report, we aimed to present a case of fatal necrotizing myositis from fulminant <jats:styled-content style="fixed-case">SDSE</jats:styled-content> infection at iliopsoas, including autopsy appearance.</jats:p>
