<jats:p>Although the severe acute respiratory illness (<jats:styled-content style="fixed-case">SARI</jats:styled-content>) case definition is increasingly used for inpatient influenza surveillance, pneumonia is a more familiar term to clinicians and policymakers. We evaluated <jats:styled-content style="fixed-case">WHO</jats:styled-content> case definitions for severe acute respiratory illness (<jats:styled-content style="fixed-case">SARI</jats:styled-content>) and pneumonia (Integrated Management of Childhood Illnesses (<jats:styled-content style="fixed-case">IMCI</jats:styled-content>) for children aged <5 years and Integrated Management of Adolescent and Adult Illnesses (<jats:styled-content style="fixed-case">IMAI</jats:styled-content>) for patients aged ≥13 years) for detecting laboratory‐confirmed influenza among hospitalized <jats:styled-content style="fixed-case">ARI</jats:styled-content> patients. Sensitivities were 84% for <jats:styled-content style="fixed-case">SARI</jats:styled-content> and 69% for <jats:styled-content style="fixed-case">IMCI</jats:styled-content> pneumonia in children aged <5 years and 60% for <jats:styled-content style="fixed-case">SARI</jats:styled-content> and 57% for <jats:styled-content style="fixed-case">IMAI</jats:styled-content> pneumonia in patients aged ≥13 years. Clinical pneumonia case definitions may be a useful complement to <jats:styled-content style="fixed-case">SARI</jats:styled-content> for inpatient influenza surveillance.</jats:p>
