<jats:title>Summary</jats:title><jats:p><jats:italic>Aspergillus</jats:italic> spp. have been shown to induce T‐helper cell (Th) 1 and Th17 subsets resulting in elevated levels of several cytokines. The objective of this study was to analyse a bundle of cytokines in serum and bronchoalveolar lavage fluid (<jats:styled-content style="fixed-case">BALF</jats:styled-content>) in patients with and without invasive pulmonary aspergillosis (<jats:styled-content style="fixed-case">IPA</jats:styled-content>). This nested case‐control analysis included 10 patients with probable/proven <jats:styled-content style="fixed-case">IPA</jats:styled-content> and 20 matched controls without evidence of <jats:styled-content style="fixed-case">IPA</jats:styled-content>, out of a pool of prospectively enrolled (2014‐2017) adult cases with underlying haematological malignancies and suspected pulmonary infection. Serum samples were collected within 24 hours of <jats:styled-content style="fixed-case">BALF</jats:styled-content> sampling. All samples were stored at −70°C for retrospective determination of cytokines. <jats:styled-content style="fixed-case">IL</jats:styled-content>‐6 and <jats:styled-content style="fixed-case">IL</jats:styled-content>‐8 were significantly associated with <jats:styled-content style="fixed-case">IPA</jats:styled-content> in both serum (<jats:italic>P</jats:italic> = .011 and <jats:italic>P</jats:italic> = .028) and <jats:styled-content style="fixed-case">BALF</jats:styled-content> (<jats:italic>P</jats:italic> = .006 and <jats:italic>P</jats:italic> = .012, respectively), and a trend was observed for serum <jats:styled-content style="fixed-case">IL</jats:styled-content>‐10 (<jats:italic>P</jats:italic> = .059). In multivariate conditional logistic regression analysis, <jats:styled-content style="fixed-case">IL</jats:styled-content>‐10 remained a significant predictor of <jats:styled-content style="fixed-case">IPA</jats:styled-content> in serum and <jats:styled-content style="fixed-case">IL</jats:styled-content>‐8 among <jats:styled-content style="fixed-case">BALF</jats:styled-content> cytokines. In conclusion, levels of <jats:styled-content style="fixed-case">IL</jats:styled-content>‐6 and <jats:styled-content style="fixed-case">IL</jats:styled-content>‐8 were significantly associated with probable/proven <jats:styled-content style="fixed-case">IPA</jats:styled-content>, and a similar trend was observed for serum <jats:styled-content style="fixed-case">IL</jats:styled-content>‐10. Future cohort studies should determine the diagnostic potential of these cytokines for <jats:styled-content style="fixed-case">IPA</jats:styled-content>, and evaluate combinations with other <jats:styled-content style="fixed-case">IPA</jats:styled-content> biomarkers/diagnostic tests.</jats:p>
